KTM was terminated in W13/14, the switch from DXB to DWC was made in W14/15.

Air Ukraine International does not exist anymore. Ukraine International Airlines (UIA) does exist, but only operates KBP-AMS, not IEV-AMS.
Also, KLM only operates AMS-KBP, not AMS-IEV.

Nothing went wrong, it is a little more complex than you may think.

Many cities have general city codes, as well as individual airport codes.

AMS - all airports: SPL and AMS, also ZYA - Amsterdam Central Railway Station
IEV - Kiev all airports, and all GDS accept this entry for KBP - Borispol and IEV - Zulhiany
MIL - Milano all airports, MXP and LIN
CAS - Casablanca all airports, CMN - Mohammmed V International
