Can anyone confirm that ADS-B is showing the altitude as per altimeter setting, i.e. local QNH below transition level and QNE (1013.25) above TL?

ADS-B is barometric altitude yes.

Technically one doesn't fly a flightlevel below Transition Altitude. So a transponder (ADS-B) showing FL015 over Ramstein (where TA is 5000ft) is actually around 700ft above ground level, unless USAF prefers to set the altimeters on their F-16s to QFE rather than QNH (if so, the aircraft is actually 1,500ft AGL (directly over Ramstein AB)).

Thats true, but like I said, the transponder only gives the altitude in relation to 1013,25.

That's not true; below Transition Altitude an altimeter is set to QNH or QFE.
1013,25HPa is QNE.
