Why fake? Wouldn't surprise me if this actually happened during a San Francisco Fleet Week.

Looking at the clearance of the F-4 to the bridge and the angle of the F-4 being upwards my first impression was fake as well.
The shadows of the underside of the bridge might give a wrong perspective.

Funny to see how we now, in the 21st century, regard anything as Photoshopped/fake which was in those days (60s/70s) considered as just "a bit of aviators fun".. Remember that regulations, air law and the strict control as we know today did not always apply back then.
Rome has Bufalotta, Arizona has alotta BUFF..

Even nowadays it is allowed to underpass the Golden Gate bridge as I did twice by helicopter in 2010 :-)

Elevation is 200 feet, we passed at 100 feet

Helo's do it all the time, lot of footage on Youtube (example: https://www.youtube.com/watch?v=yIp9ux3dOW8)

Small aircraft also do it frequently, fast jets haven't recently and I don't think they're allowed to anymore, but I'd say that regulations forbidding it haven't been in place before somewhere in the 70's.

I'd say it's real.
